If you have applied for one or more examinations offered by the City of Rochester, New York State or other County Civil Service Commissions on the same date, you must complete a cross filer form as well.

Veterans wishing to claim additional examination credits as a veteran or disabled veteran of a period of war or conflict as prescribed by the Veteran’s Administration must file Form VC-1 and/or Form VC-4 along with a copy of their discharge papers.
If you have moved and have applied for a Civil Service Exam or have recently taken an exam and are awaiting results, please complete the Change of Address Form. Be sure to fill out the form completely, so we can accurately update all your exam records.
