Recovery Connection Overview

Recovery Connection is a case management service of the Monroe County Office of Mental Health. Case managers are available through Catholic Family Center, Restart Outpatient and ViaHealth Behavioral Health Network-Rochester Mental Health Center to clients in any chemical dependency outpatient treatment setting in Monroe County. This program is designed to improve treatment outcomes for individuals who frequently receive detoxification or other intensive levels of service, but are not successful in linking to any sustained treatment program. Retention in treatment is associated with better outcomes.

Benefits of participation in the program include: housing assistance, transportation assistance, benefits advocacy, legal assistance, motivational coaching, and support in linking to primary health care, mental health, and vocational services.

Who is eligible for Recovery Connection?

Monroe County Office of Mental Health criteria for eligibility are:

  • An alcohol or drug dependence diagnosis of at least two years’ duration
  • Three or more admissions to detoxification or inpatient treatment in the past 12 months, or four or more admissions in the past 24 months
  • Adults 18 years of age or older
  • Resident of Monroe County
  • Meets eligibility for Medicaid insurance

What are the expected outcomes?

Our case manager, together with participants and providers, will achieve the following desired outcomes:

  • Successful engagement with a treatment provider
  • Discontinued use of alcohol and drugs
  • Successful completion of treatment
  • Improved employment status
  • Reduced Medicaid costs
  • Reduced reliance on public assistance
  • Decreased days of homelessness
  • Decreased days in jail or prison
