Birth Certificates

Monroe County Vital Records issues only certified copies of birth certificates for persons born in Monroe County – Rochester, New York.

We issue two types of birth records:

Certified Transcript of Birth: **RECOMMENDED FOR PASSPORT/DRIVER’S LICENSE** Transcripts are issued to all persons over the age of 18 in New York State. The document includes parent's names and is valid for all forms of identification with the exception of dual citizenship.

Full Form: The full form of the birth record is always issued to minors. It is also available to anyone born after 1928. This record is a certified copy of the image stored on file with the state of New York. The images vary year to year and are hand written prior to 1965 in most cases. Quality of these records is not guaranteed. The record includes parent's names and time of birth. This record is valid for all forms of identification and is the only record accepted for dual citizenship.

You must know which type of document you need to purchase. No refunds or exchanges will be granted based on selection of the wrong document.

Who Can Order?

You may order copies of your own Monroe County birth certificate, but only parents-of-record or a legal guardian (showing a Court order) may obtain someone else's record. Anyone else must present a notarized letter of permission from an authorized person.

Documentation of guardianship or power of attorney may be faxed to (585) 753-5961. You must include the order number at the top of the document in order for the records to be attached to your order. Failure to provide appropriate documentation will result in the order being delayed and/or cancelled. The Vital Records Office will not be responsible for orders being delayed due to incomplete or invalid information.

Death Certificates

Monroe County Vital Records issues certified copies of Monroe County death certificates of individuals who died within Monroe County, which can typically be used to obtain death benefits, to claim insurance proceeds and for other legal purposes.

Who Can Order?

Only a surviving spouse, parent, child, sibling or attorney (for deceased or someone making a claim against the deceased) is automatically entitled to a Monroe County death certificate. All others must provide documentation of need for the record or present a notarized letter of permission from an authorized person.
