Description of Duties

This is a community outreach position in the Monroe County Public Health Department responsible for making home visits to increase health care awareness in areas such as pediatric care, rabies, and lead contamination where duties involve providing basic information regarding health care and assisting with obtaining health care services provided by the Department, or visiting people afflicted with Tuberculosis (TB) to monitor the taking of medication.  Duties and work schedules vary depending on assignment. Employees are hired to work in either a health care awareness position or TB position. The employee reports directly to and works under the general supervision of a Nursing Coordinator, Registered Professional Nurse or other higher-level staff member.  Does related work as required.

Minimum Qualifications

Graduation from high school or possession of an equivalency diploma, plus EITHER:

(A)  Two (2) years paid or volunteer** full-time or its part-time equivalent supervised*** experience working with young children (under five [5] years of age) and/or parents such as working in a day care, teaching, or assisting with teaching; OR,

(B)  Six (6) months paid or volunteer full-time or its part-time equivalent experience working in a community setting with ‘high risk’ individuals such as the economically disadvantaged, the uninsured, refugees, the elderly, the homeless, those with human immunodeficiency virus (HIV) and other chronic health conditions, mental illness, and inadequate housing and education; OR,

(C)  Graduation from a regionally accredited or New York State registered college or university with an Associate’s, Bachelor’s or higher-level college degree; OR,

(D)  An equivalent combination of education and paid or volunteer** full-time or its part-time equivalent experience as defined in (A), (B) and (C) above.
