Since late March, the Monroe County Clerk's Office has continued to adapt DMV services to keep them accessible in a socially distant maner. Currently all drop boxes are for plate surrenders only. All other transactions please mail to the address below.

Surrender License Plates/Cancel Registration
To cancel a vehicle's registration and remove it from your insurance policy, you can use any drop box location or our mail-in address to surrender your plates.
Your plate surrender receipt will be mailed to you. Plate surrender forms will be available at the drop boxes or can be downloaded below:
Please note that plate surrenders cost $1, payable by cash or check to "Monroe County Clerk."

The same process is being used to re-register a vehicle you already own, for example, a "summer car" you took off the road for the winter, with a few key differences:
- It is not necessary to submit your title if you already own the car and are the only person on the title (see the packet linked above for more detail on this point)
- If you are already the titled owner of the vehicle, we do not need a sales tax form or bill of sale -- these documents are only needed to change ownership of a vehicle
- If you requested a credit when you surrendered your plates for the season, the registration is still unexpired, and you are putting the same vehicle back on the road, no fee is required. If it is a different vehicle, we require a $10 transfer fee. If you need to renew the registration, the fee can be determined using the fee calculator linked in the full packet.

Replace a Lost/Missing/Destroyed Title

Complete By Mail: Please note that while you can apply for a replacement title through the mail with a local DMV, you will still receive the title directly from NYS DMV in Albany.
- Complete DMV Form MV-902, “Application for Duplicate Title”
- Provide copies of proof of your identity. NYS DMV uses a “point system” and we need six points. A copy of your NYS license, permit, or ID card is worth six points. If you don’t have this document, please see this Proofs of Identity chart to find other documents that are worth six points
- Include payment of $20 by check or money order, payable to “Monroe County Clerk”

Renew a License
Please note that license renewals require an eye exam. The eye test provider must either submit the eye test electronically to DMV (ask your provider if they participate) or provide you the results of your exam on the form below.
Complete By Mail: submit the following items:
- Renewal Notice - Complete the renewal notice you received by mail or click here to download and fill out form MV-44
- Vision Test Report - If your provider doesn’t participate electronically, include form MV-619 as described above
- Fee - Include your renewal fee as described on the renewal application mailed to you. Make the check or money order payable to “Monroe County Clerk.” Renewal fees by license class are:

Add $30 to any of the above fees if your license is Enhanced. (There is no additional fee to renew a REAL ID license.)

Please note that while you are required by state law to update your address with NYS DMV, you are not required to replace your documents with one showing the new address, although if you desire to, the form above includes information about the fees required to do so. If you choose to order a replacement document, please make your check payable to “Monroe County Clerk.”

Registration Renewal
Note: If your registration has been expired up to 1 year, it is still processed just like any other renewal. If it has been expired for more than one year, please follow the instructions for a new registration.
Complete By Mail:
- Complete Form MV82, Vehicle Registration/Title Application, or the renewal notice you received in the mail.
- Provide your renewal fee, which is either on the personalized renewal notice you received or look up the fee on the NYS DMV website. An additional County use fee, used to support the bridge and highway fund, is collected based on the weight of your vehicle - $10 if it weighs 3,500 pounds or less, $20 if it weighs 3,501 pounds or more.

Please make sure your address is up to date with DMV before ordering as the replacement document will be sent to whatever DMV has on file. See information elsewhere on this change regarding how to change your address with DMV.
Complete By Mail:
- Replace License, Permit or ID Form (MV44) - Provide your DMV ID number if you have it, but a replacement can be processed without it.
- Replace Lost Registration Form (MV82)
- Provide copies of proof of your identity. NYS DMV used a “point system” and we need six points. A copy of your NYS license, permit, or ID card is worth six points. If you don’t have this document, Please see the proofs of Identity chart to find other documents that are worth six points.
The fee for a replacement license/permit/ID is $17.50. The fee for a replacement registration document and sticker is $3.

Order Your Driving Record (a.k.a. Abstract)
This document is required by some employers and can also be very useful in determining why a license is suspended (e.g. in what courts you have an unpaid traffic ticket).
Complete By Mail:
- Complete Form MV-15, Request for DMV Records
- Provide copies of proof of your identity. NYS DMV used a “point system” and we need six points. A copy of your NYS license, permit, or ID card is worth six points.
